ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
Care Manager
· Responsible for a designed group of residents during the shift; knows where their residents are and physically checks on them throughout the shift.
· Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participion in activities.
· Responds to security system and resident call bells promptly.
· Notifies supervisor and/or Health Care Coordinor if a resident has increased care needs.
· Assists with continence management and disposes of all continence products properly to ensure sanition of resident suite and community restrooms.
· Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.
· Maintains cleanliness of resident’s room and work areas.
· Practices good standard care precautions of cleanliness, hygiene and health.
· Helps residents maintain independence, promotes dignity and physical safety of each resident.
· Actively participes/leads and assists residents with activities of daily living (i.e. bhing, dressing, toileting, grooming, ambulion, transferring, eing) as instructed.
· Engages residents in life skills and other life enrichment activities.
· Greets and assists all internal and external customers, guests, family members, residents, vendors and team members.
· Does resident laundry as assigned and needed.
Medicion Care Manager
· Reviews service plan to learn pertinent informion about residents.
· Assists/observes medicions and trements for each resident using the medicion observion record and the Six Rights of Medicion Pass.
· Documents and initials form as medicions are given including approprie documention for refusal or missed doses.
· Maintains confidentiality of all resident informion including resident medicion.
· Reports all resident concerns made while assisting resident with the medicion to the Nurse and/or Health Care Coordinor (HCC).
· Restocks medicion cart after all medicion passes.
· Assists in checking medicion regardless of packaging system.
· Counts all narcotics with another Medicion Care Manager or Nurse each shift.
· Maintains and cleans the Medicion Room, med charts, trement carts for neness cleanliness, availability of medicions and expired medicions.
· Follows re-fill process for medicions.
· Participes in the development of the Service Plan and monthly updes.
· Takes and records temperure, blood pressure, weight, pulse and respirion res.
· Reports occupional exposures to blood, body fluids, infectious merials, sharp sticks and hazardous chemicals immediely.
· Other duties as assigned by the Supervisor.
QUALIFICATIONS

Educional Requirements and Experience:
· Certified Nurse’s Aid certificion preferred.
· High School diploma/GED
· Must be 18 years of age.
· CPR Certificion preferred
· First Aid Certificion preferred
· Previous experience working with seniors preferred.
Knowledge, Skills and Abilities:
· Language Ability:
o Ability to communice effectively speaking the primary language of the residents.
o Ability to read and interpret documents such as safety rules, opering and maintenance instructions, and procedure manuals.
o Ability to write in required logs and routine correspondence.
· Mhemical Skills:
o Ability to add and subtract whole numbers.
· Cognitive Demands:
o Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
· Competencies:
o Must demonstre an interest in working with a senior populion.
o Interacts with guests, residents and staff in a courteous and friendly manner.
o Responds promptly to resident needs.
o Supports organizion's goals and values.
o Balances team and individual responsibilities.
o Ability to make choices, decisions and act in the resident’s best interest.
o Ability to remain calm in difficult situions.
o Ability to handle multiple priorities.
o Competent in organizional and time management skills.
o Must demonstre good judgment, problem solving and decision making skills.
ENVIRONMENTAL ADAPTABILITY
· Works primarily indoors in a clime controlled setting.
· Team Members may be exposed to blood and/or body fluids with potential exposure to hazardous merials and infectious diseases.
· Possible exposure to unpleasant odors.
· Possible exposure to chemicals as identified in the MSDS Manual.
PHYSICAL REQUIREMENTS
The physical demands described below are representive of those th must be met by an individual to successfully perform the essential functions of this job. Reasonable accommodions may be made to enable individuals with disabilities to perform the essential functions.
While performing the duties of this Job, the Team Member is regularly required to stand and walk. The Team Member is frequently required to use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is occasionally required to stoop, kneel, crouch, or crawl. The Team Member must regularly lift and /or move up to 50 pounds independently and up to 200 pounds with assistance of a second employee. Must be able to push up to 350 lb. (wheelchair). Specific vision abilities required by this job include close vision and distance vision.
